Since the skin on your back an be pretty resilient, the first line of defense is to use a back brush daily when showering. This will help exfoilate the dead skin cells preventing them from accumulating in the pores. A blemish is caused by an accumulation fo oil and dead skin cells that act as a breeding ground for the P-acnes bacteria. My favorite product for blemishes on the back is DDF PUMICE ACNE SCRUB. A combination of circular beads to exfoliate in a creamy formulation with 2.5 % Benzyol Peroxide to attack bacteria. This is truly the BEST product you could use for back blemishes. If you are already taking antibiotics from your doctor, these should help your back too.
